NOTICE RE: TAX STATEMENTS REPORTING HEALTH COVERAGE

As an eligible Beneficiary of the Trust, you may receive a hard copy of your IRS Form 1095-B statement for the tax year from the Trust Office, upon request.  The Trust Office uses Forms 1095-B to report to the IRS eligible Beneficiaries and their family members enrolled in minimum essential health coverage from the Trust. Individual taxpayers no longer need the information on the Form 1095-B to prepare and file their individual returns. Therefore, the Trust Office is no longer mailing these forms to all Beneficiaries.  If you would like a paper copy of your Form 1095-B mailed to you, you should send a written request to the Trust Office via email at rehncdh@rehnonline.com or via mail:
